There are 10 Centres that have been identified for issuing PWD certificate for 50% All India Quota and Central Universities and Institutions. One of them is Madras Medical College. Page 73 of https://www.digialm.com/per/g01/pub/1258/EForms/image/pdf2/Information_Bulletin_NEET_MDS_2020.pdf has the template. You need to visit Madras Medical College and ask for Disability Certificate Board department or anyone from that department who issues those Certificates. The Board will conduct various tests and assess you based on the regulations. You can meet the Medical Superintendent who can help you with the process.

Different colleges and exams have different cutoffs and you can only get a seat in the college if you satisfy its respective cutoffs . Here i am unable to predict your chances as you haven't mentioned your exam name. If it is UPSEE exam , then yes you can surely get some good colleges . But still i am unable to predict the colleges as different colleges have different cutoffs for different courses.

You must not give in to this pressure. Don't worry. You should definitely put in all your efforts and hard work and appear for the exam. Even if you don't get satisfactory results, work harder and smarter. Learn from your mistake and don't stop untill you have achieved your goal.
If I say any more, you may think that I am a random motivational speaker but that is not the case. It does't matter if you have passed from a random college and never think that you have not done anything significant.
You have your age on your side and utilize it. A large portionn of Indian youth are unemployed and those who are employed, of them, a lot do not enjoy the work they do. But that is not the point.
I think you should put in your bet efforts. You still have time to prepare for the Prelims and I think if you chalk out a plan and strategize like playing to your strengths, you will definitely accomplish your goals.
And if you fail, well what better way to learn ? Don't feel low, go for a walk or take some rest. After refreshing your mind, put in all your efforts and concentration, and I think you will definitely succeed!

Special Round counselling is held only after all the official rounds are over. This special round is meant for filling of vacant seats that have not been occupied even after all the rounds are done. This happens usually when:
- The last round candidates choose to not freeze a seat
- They withdraw
- Extra seats are added (rarely)
The point is that special round counselling is sort of a game of chance - you cannot depend on getting a better seat after withdrawing from your own. If you are lucky enough then maybe you can get a seat of your choice in your choice of branch but it is a huge gamble.
I'd personally suggest only go for it if you are prepared to take any seat that would be offered in the special round or you are already planning to reappear for your exam next year anyways. Otherwise, to be on the safe side, freezing your seat in the last round could be a better choice.
Decide prudently, do not rush it.
