Well 4 lakh is not a very good rank, but according to past year's trend, there are chances that you might get into a private medical college for MBBS, not in very 1st round but till 3rd round or stray vacancy round you might get into a private college for your desired course. As per your mentioned query, yes you can start your NEET preparation. NEET syllabus comprises of both class 11th and 12th syllabus. So you can start with basics of your class 12th. Many students also take coaching for exam and some of them start their own.
