cut off marks / rank for medical colleges keeps changing every year depending upon various factors like number of students that appeared in the neet examination, kind of marks obtained by students in neet , toughness level of the neet examination, number of seats available etc.
So , you should try to get at least 50 marks more in comparison of Neet 2019 in your Neet 2021 Examination to be on the safer side, so you should try to get at least around 425 marks.

Dr. NTR University of Health Sciences, Vijayawada will release the cutoff of AP MBBS after the completion of the AP MBBS 2021 Counselling Session. The cutoff will include the ranks and NEET 2021 marks obtained by the candidates belonging to the reserved and unreserved categories to secure seats in the All India and State Quota. Directorate General of Health Services (DGHS) will declare NEET cutoff 2021 AP for 15% All India Quota seats.

In last 2-3 years, the trend is changed and now to get admission in a best government medical colleges you must have secured 650+ marks in NEET no matter which catagory you belong.
You may have got admission at 550+ also but chances for that is very less.
Less than 550 marks will rarely help you getting a seat for MBBS in any good college.
You should try to get as good as possible marks. Keep your preparation of top level and set your aim high.

You need to appear for NEET first. The cutoff for NEET is decided on the basis of the number of candidates who appeared in the exam and how tough the exam was. Sri Venkateshwara Medical College has a total of 15% Seats for All India Quota counselling and the seats are reserved under following Categories with seat Matrix(2018): GN –25, SC –03, ST –01, Others -01.
The Remaining seats of 85% MBBS for Sri Venkateshwara Medical College shall be allotted through State Quota.
The cutoff for students who belong under the AIQ, based on pervious date is from around 44,000 to 54,000 rank.

Sorry to say but there is no option for applying process in government medical colleges before NEET examination. This examination conduct for fare selection of candidates. If you are qualifying the NEET examination with good rank then you can get government medical college through counselling. If your rank medium then you will get semi government medical college and if your rank is less than that you will get private medical college. Actually there All India counselling and State counselling are conduct different counselling. So you can't apply before the Examination in government medical college.

First you need to understand the Syllabus and important topics. Your aim to score 340-360 marks in (zoology + botany) biology, 160-170+ marks in chemistry, and 150-170+ marks in physics. To order to understand the NEET Syllabus. You have to complete the NCERT of Physics, Chemistry and Biology. Make Notes during preparation that will help you in the time of revision. Solve previous year paper that will help you to manage time and accuracy. Revise Daily.

Let me get back straight to your question, yes, 400+ may get you government college but not limit yourself upto just little more than 400 .
As you have mentioned your home-state as Telangana, so we look at the last year minimum mark or say it cutoff for a SC student to get a government college then it was 426.
Just one or two more marks may or may not fetch you a government college as cutoff varies every year depending upon difficulty level of paper, number of students sitting in exam, and number of seats. As by the tone of your questions it is clear that you still have attempts for NEET, so I will suggest you to score something around 460-470 to be on safer side and to get a good government college.
