Since you know that the cut-off varies year to year it's not that easy to predict which college one can get into precisely. Looking at the previous year's cut-off of MP state, the last rank who got the seat was 48715 in general rank which was 555 marks. Keeping in check the previous year cut off the probability of you getting a seat is high.
Yes you can get into colleges in Bhopal, and Indore, but do the counseling sincerely, maybe you won't get it in 1st round, don't lose hope, and try till 3rd round.
Some of the colleges I think you can apply in these marks are :
1.MGMMC Indore
2. Gandhi Medical College, Bhopal
3. Bundelkhand Medical college , Sagar
4. Shyam Shah Medical College, Rewa
You should also keep dentistry an option as backup and for that GDC Indore is highly prestigious, check out the official websites of these colleges.

Looking at the previous year NEET cutoff, in order to get admission seat in Government Medical colleges for MBBS admission, you will need a NEET marks of around 670-690 in NEET as general category. So, you have very little or difficult chances of getting a government medical colleges for MBBS or BDS with the above mentioned NEET marks.

If we look at the previous year's cut off for the BCE Muslim Minority category for government medical colleges, it was 640 marks and the college allotted was Gandhi Medical College. So your chances of getting a government seat are extremely low.
However, you might get a seat in the following Muslim Minority private colleges in Telangana-
Shadan Institute of Medical Sciences, Hyderabad- 386
Aayan Institute of Medical Sciences- 355
The Non- Minority medical colleges had a minimum cut-off of 425 marks.

The last PWD seat to be allotted in Telangana through state quota was to BCA category and the marks scored were 145. You might have a chance of getting a government seat, however, they are very low.
In NEET 2021, the OBC PWD category candidate got allotted Government Medical College, Barmer at 130 marks.
