I can understand your concern as I'm also a north indian students studying in medical College of Kerala.
Don't worry much about that, you are not gonna face much problem here, there are two major concern here coming to Kerala is language issue and food habits. All other things are pretty much good here.
In academic things you gonna never face issues as all teachers will be using English as teaching language and for communicating. Although you will have to learn the local language as you will have to interact with patients and most of the patient will be knowing Malayalam only if you go in some district.
Also you must be knowing Kerala is most literate state so in educational basis they are very standard and they don't compromise with that, it's means if you come here certainly you will get best kind of medical education in most of the top and average medical College in Kerala. Although here are also some college which is below average.
To avoid major problem language issue and food, try to take college in major cities of Kerala where north indian population are found such as kochi, trivandrum, Calicut/Kozhikode, Thrissur etc these are major cities where you can choose your college where sometime you may found people with high socio-economic status who will be knowing English or Hindi language. Also in these cities you will find north indian resturant so you can get some taste on weekends.
In all other things, there are no any other issues i had faced here. Ragging here is in minimal amount as compared to North Indian college. Also from teachers to students or local people all are very kind and generous people, they are one of most humble people's in India.
So i don't think you gonna face any major problem if you see academically although you will be facing some minor issues which you will have everywhere when you go to a new city.

According to the score you are expecting you are eligible for getting a semi government college in Gujrat. The cost of semi Government medical colleges depends upon the seat and rank what you will be getting. The cost varies from 5L to 8L per annum.

Yes, government medical colleges in West Bengal do have college canteen and cafeteria. Usually gym is not present in the premises of government colleges.
Private colleges like JIMS hospital and medical college provide gym facilities within the campus with cafeteria as well.

See the central government of India has made it mandatory for all the engineering and medical colleges (government) to have a 10% quota for the Economically Weaker Sections of the society, which is the EWS quota. So, as a result, all the government medical colleges of Mumba have EWS quota for such students and you can take it's benefit.
