1) Aadhar Card(Most preferable)
Or
2) Voter ID card
3) Pan card
4) Driving licence
5) Ration Card
6) Passport

Yes,you can submit aadhar card photo id or pan id as photo id proof as per your preference in mp neet ug counselling registration.
to upload id resize it by cropping make it as mentioned in the form.it can be in jpg form.

When you deposit registration fees for NEET UG counseling, it is a one time deposit. You don't have to pay any kind of fees post registration.
What kind of change do you want in your counseling registration? If it is about choice locking then you are allowed to make changes but once you submit your choices, you can not make any change till second round.
If you wish to make any change in your NEET application, you can't do any change once result is out.
But yes, no extra fee is charged once you have deposited registration fees for counseling.

No, it's not compulsory but keeping aadhar card with you is a safest option for counseling. Whatever ID proof you have mentioned you can carry that with you in case of doubt otherwise it's not a compulsion to use same ID proof. Any government authorized ID proof will work for you.

Following is the Domicile criteria for Madhya pradesh for NEET candidates-
• Only the candidates who are domicile holders of the state of Madhya Pradesh are eligible for the 85% state reservation in MP NEET . However, some other candidates who are eligible to avail the same are:
• Candidates who are children of Freedom Fighters
• Candidates who are wards of Defense Personnel
• Candidates whose parent/s work with the Govt. of India and are posted in Madhya Pradesh.
Since your parents have Domicile certificate of Madhya Pradesh, you can too avail the Domicile of MadhyaPradesh. Before NEET counseling begins, kindly get your Domicile certificate made and signed by a competent authority.

To be an eligible for domicile of MP, you should have passed 10th and 12th from Madhya Pradesh or the same state. Your eligibility for 85% state quota will depend upon your Domicile. And if you have done 10th from Madhya Pradesh and 11th, 12th from Rajasthan then you are not eligible for the domicile of MP . You might not be eligible for the MP state quota for NEET.
