Well dear,the only things you need to present at the time of counselling among all the things you have listed will be an identity proof document(Aadhar,PAN,Passport,Driving License) and the domicile certificate for the category details.

The cutoff rank for ST candidates in NEET UG 2020 is 146-113. Your score is above the minimum cutoff required for eligibility. However, it is very difficult to get a medical seat even in any private college as you marks is just around the margin. You can try through management quota though if you strongly want to get admitted to MBBS course. Even then, getting a college of your choice is almost very difficult.

Since you did not mention your category, I am assuming you are of general category. Last year the cut off for Patna Medical College through 15% All India Quota was a rank of 1973. While the cut off for Patna Medical College through 85% State Quota in Bihar was a rank of 4552.
So based on this, you have good chances of getting Patna Medical College of you apply under 85% State Quota in Bihar.

Congratulations on clearing NEET. I believe having a score of 625 and AIR of 10k, secondly having the EWS Advantage, you might have slight chances in making it to AIIMS Raipur. Besides AIIMS, there are many Top Government and State Medical Colleges, and you should also consider them besides targeting AIIMS Raipur.

To be frank you will not get jubilee mission medical college of Kerala. there are very less probability that you will get a seat in self financed college in Kerala. Last year cutoff rank for all india merit quota in Kerala State was about 40k. And your rank is almost double of that. So from my side you had no chance to grab a seat in either Kerala or Karnataka Private colleges which holds low fess because a high number of candidates from all over India apply in this two. You can easily grab best Private medical colleges of bihar and up. Try in thsese two states. You can check cutoff rank for Kerala colleges from link below (note it is state rank given in that data ).

So the difference in terms is :
Government -Is a body in which you have elected representatives which works for the welfare of its citizens.
Trust- Trust is when a person holds property for someone else. (Kisi aur ke liye tum kaam karre ho)
Society- when we talk about society in broader sense, collection of small small colonies forms a society. A society may have different customs,rules,regulations etc. Basically an association which works to achieve a purpose may be called a society.
But that does not happen.in case of government a government is a statutory body which works for its citizens.
Government-society:
Government works for welfare of societies. And societies are collection of individuals/families residing together at a place.

It's quite difficult to predict that whether you will get seat or not as cut off marks for medical colleges keeps changing every year depending upon various factors like number of students that appeared in the exam, kind of marks obtained by them , toughness level of the examination etc.
You have not mentioned your domicile state but yes when we go through previous trends then it suggests that with 253 marks your chances for Government Medical College is very low.

The cut off of institutes vary every year based on various factors so an accurate prediction is not possible. You have chances in AIQ as well as state quota seats based on previous year data. You have chances in AIQ and state quota seats of following colleges
You also have chances in of BDS colleges
If you are looking for admissions in CMC vellore then you might have chances in the college. You have to register for CMC vellore MBBS 2020. The last date was March 26. After the declaration of NEET scores the registered candidates have to login to enter their result details such as marks obtained.
