So on such marks you have very low or almost no chances to get any of the government medical colleges under state quota of UP as the closing rank for EWS was around 14-15kfor EWS category . But on such score if you don't want to go for private colleges then you can go for some deemed institutions but yes fee will be bit higher as compared to that of government medical colleges.

With reference to your query, I would glad inform you that at your marks your chances are very low of getting seat in government catgeory as cut off generally goes above 450 in government colleges and even in private colleges their are very less chances as cut off for private colleges also goes above 350. I would suggest you to start preparing for next attempt

At this rank in OBC category there is very tough for you to get any AIIMS.
Now if we talk about govt college then from AIQ counseling also it's seems very tough for you.
Now as your profile suggest you are from kerala so state counseling cut off marks for last year.
You haven't mentioned your subcategories in OBC as there are many in Kerala State.
Different categories cut off marks
Bh- 611 marks BX-576 marks DV-545 marks ez-606 marks
Mu-611 marks la- 585 marks and vk - 598 marks.
There are similar ranks to last year as of now which I'm hearing. So check your subcategories cut off from above and then you can decide.

I hope you are doing well. I see that you are expecting 145 in NEET UG examination, it is a bit of task for you to get a seat in private colleges even if you are obc. And it would be difficult for you to get a seat in Jamia Humdard. Anyways the fees here is approximately Rs 12.5 lakhs for the first year. Anyways you could get a seat through management. I see that you are from Delhi, here are few of colleges you can apply for MBBS in Delhi and Uttar Pradesh.
