As you belong you obc category so you have good chance of getting admission in any government college but it only depends upon the cut-off marks of government colleges and universities. But as per the previous year cut-off you have chance of getting admission in. Now, it is important to know that ranks and cut off change every year depending on the difficulty level of the paper, the number of students qualifying and the number of seats available.

If you get a seat in first round and don't want it then you can go for free exit.
Free exit means if you get a college and you don't report to the college then you can exit from the that round of counseling.
If you get a college allotted in second round you must take admission in allotted college or you will have to loose your security deposit.

You haven't mentioned your category assuming that you belong to general category you do not have chances to qualify in NEET UG as per previous year cut off. The cut off for general category was 147-720 marks. You have to qualify in NEET UG in order to get admission in MBBS, BDS and Ayush courses. But to get seat in a Government or Private college you need to score above the cut off marks. Ideal score for general category candidates is 590 marks or above and for reserved categories it varies in between 490-550 marks or above. Even if the cut off decreases you do not have chances to get seat in Private or Medical college this year. You have to take a year off and prepare for NEET UG 2022 and score the above mentioned marks. cut off changes every year depending on factors such as total number of candidates appeared, difficulty level of examination and total number of seats available etc.

As your profile suggest that you are from bihar. So I'll answer according to that.
In all india Ayush counselling you have almost no chance of getting BAMS ( last year cut off 528 marks) and BHMS ( cut off 505+ marks).
Now if we talk about bihar then there is only two BAMS government college one is in patna patna having 125 seats and another in begusarai 30 seats. So due to less number of seats. It's very difficult to get seat in general category at your expected marks. Same goes for BHMS course also.
Although you can try for BHMS course through state counselling. In that you may have little chance.
You may get good private BAMS/BHMS college in different state at your marks.
