No, there is no need of filling any any form before the commencement of NEET exam 2021. The application form for this year has 2 phases. In which phase 1 of the application form has only released by the NTA.
As per the regulations and guidelines of NTA, the phase 2 form will be released after the completion of NEET UG 2021 and before the commencement of the councelling, where you will attach all the most necessary documents such as 10th marksheet, 12th marksheet etc.,

First of all, congratulations, you've done really well! 450 seems like a decent score. If the cutoff goes down, then you might be able to secure a seat at a govt college, but a private college is almost sure. However a seat through AI quota seems a bit difficult!
You will either get Deemed University or Private College. With the score of 500 you can get the Best Deemed University and Best Private Medical College.
Note: This answer is given as if you are from General category or OBC.
If you are from SC / ST category than you will get the good government medical colleges.

Yes of course you can change your category in NEET Application form during this NEET Correction window which will remain active till August 14th.
So If you want to make any correction in your NEET UG 2021 Application form related to your category or any other editable detail then you can do so during the NEET correction window which is now live from August 11th to 14th 2021 as per the revised dates and you can follow the given steps to make correction in it :-
** Visit the official site of NEET 2021and click on the button saying' Correction in NEET UG 2021 Application form'.
** Now login using your login credentials.
** Edit/delete/modify the details whichever required in NEET 2021 application form.
** After making corrections just click on 'Preview and Submit' and then preview your form if it does not require further modification then just submit it.
** Now you will get an OTP on your registered mobile number and e-mail, just enter it and your correction will be submitted.
** Now take the print out of your NEET correction slip for future references.

PHYSICS is the toughest part for most of the medical aspirants as it has both numericals and concepts to deal with ,so keep at least 4 hours each day for physics
In Chemistry you have to deal with reactions and learn them and solve questions as well, so give at least 3 hours to Chemistry
