Neurosurgery is a very complex simple and time consuming procedure is not possible that neurosurgeon visits 3 to 4 hospitals daily. Actually a neurosurgeon remains in contacts with various hospitals and is called for a surgery according to the situation. The number of visits can be either two or 3 at maximum per day and could be 0 at some days. So 8 hours per day as you have mentioned includes both surgery and visit time, which they manage according to the time a surgery takes.

It depends on the background which that doctor comes from And the type of doctors he is for example some specialists like Cardiologist earns 2-3 lakhs on daily basis so that means they can easily afford.

Becoming any sort of doctors require years and years of hard work and most of all patience. Because it is a time taking process and a long journey. Especially for neurology because I'm order to become one in neurology because you first have to complete your 10th with physics, chemistry and biology. Then get and MBBS degree and M.D by qualifying various state level or national level entrance exams like NEET. Then you have to give time for internship and training. In order to become a neurologist you have to meet the following eligibility criteria :-
1) you first have to complete your 12th with subjects like physics , chemistry, biology,etc.
2) you need to earn and MBBS degree. From a good college or university.
3) you will also need a M.D with 50% in MBBS.
4) you need to have doctor of medicine for three years.
5) Internships and training.
In order to get into MBBS course and PG level course you need to crack entrance exams like NEET or AIIMs entrance exam, etc.
