You did not mention whether you want admission through 15% AIQ or 85% State quota. You also did not mention your category. Assuming you want admission in your state (Assam), here is the tentative cut off for the medical colleges for UR seats: (AIR)

Under BC category with 514 marks, yes you stand a chance of getting admission in Government colleges in tamil nadu. The last cutoff marks for colleges have not gone below 450. So you have a slim chance based on the number of seats but you do surely stand a chance.

It's tough to get on all India basis , but still.
You can try state counselling to find a government or semi-government or private college in mbbs, but I would suggest you to even try Dental and ayurvedic, so that you get a relief and even Vet.

As confirmed from recent media accounts, Karnataka government has decided reserve 50 % seats in Post-Graduate (PG) medical courses for students from Karnataka under the Open Category at state’s private medical colleges from the academic year 2019-20.
At present, 42% seats are of private medical colleges, termed as Open category and now of that 42 %, 50% seats will be reserved for students from Karnataka.
According to the seat matrix of Karnataka PG medical admission process, 33% goes to the government, 42% for private colleges and 15% NRI and 10% institutional and in-service quota.

As you said you are from Bihar, you do not have a domicile certificate for the state of Uttar Pradesh. Only domicile holders of a particular state are eligible for the state counselling (85% state quota) of that state. Rest of the 15% seats are available through All india quota
So you have two options-
1. If you want a government college, you must apply for All India Counselling and lock the choice of college you want
2. If you are willing to take admission in private medical colleges of UP, you can register for state counselling there (selecting your state as other state ). Note that you will only be eligible for management seats in private colleges.
