There are two ways by which you can get admission in a private medical college.
Firstly, through NEET, NEET-qualified candidates need to apply through the respective state authorities for admission. Applications are generally available in online mode. The typical application process through state authorities is explained in the link below:

Another way is you can directly apply for the private colleges counselling form where ever you might like and if your rank meets the criteria of that particular college you can take admission in that.

The EWS category is an official reservation category that needs to be considered for admission into every college. So college consider EWS category also as other category. You can surely apply for private medical College.
Most of the colleges doesn't have fees as per the reservation. The fees seems to be same for every student. There might be some college that may offer scholarship to reserved category students .

As per last years cutoff, getting a MBBS seat through all India quota would not be possible. Cutoff is also expected to increase, and as per last year cutoff your rank is already higher. You might get a MBBS seat though state quota and BDS through both AIQ and state quota. I will some of the colleges you might get through state quota for a MBBS seat.

Round 1 has free exit, hence there is no fees for upgradation. If you report to the allotted college in Round 1, you have to submit willingness for second round up-gradation as Yes. If you desire to take part in second round of online allotment, you will have to submit fresh choices during choice submission period and lock the choices during period of locking. Once you join second round seat, you will not be allowed to vacate from allotted 2nd round seat.

Kerala registration for private medical colleges have been closed. It was opened for two times i.e, once in Feb 2020 and other after neet2020 exam. Now there is very less chances that they will open for new candidates. You can visit KEAM 2020 for Kerala counseling website and after opening that go to candidate login portal then it will open to main counseling page where you can login if you are registered already or you can get other information like last year cut off ranks, new notifications, prospectus, college fee if it is released and many other info. Keeping visiting the site and check notification section to know if registration are opened or not.
