In India if you want to become a surgeon,firstly you need to prepare for the medical entrance exam which is called National Eligibility cum Entrance Test (Undergraduate) or NEET (UG) to study MBBS(Bachelor of Medicine and Bachelor of Surgery) . The exam is conducted by NTA(National testing agency).This exam has multiple choice questions and there is four marks for every correct answer and one fourth marks is deducted for every wrong answer.One needs to qualify the neet exam to be eligible for counselling for the neet exam.There are huge number of aspirants giving neet every year and those who score good, above the cutoff level bags a seat in government medical colleges according to their rank.
The syllabus of neet is similar to that of +2 level syllabus and the preperation is taken during these days for the exam.For preparing the neet exam one must study hard and practice previous year papers.
After cracking neet exam, one gets into a medical college for their ug studies in medicine.MBBS is a five year course and one studies about Pre-Clinical ,Para-Clinical and Clinical.It is a tough course to study with huge amount of syllabus but if one is persistent about their studies and dedicated then one could easily ace it.
After 5 years of MBBS you get the bachelor degree in medicine.In order to become a surgeon the next step is to crack neet pg exam to study MS(Master of Surgery) which is a three year post graduate course to study to get the license to do surgery in India.

This totally depends upon the doctor and the fee he/she charges for the surgery. Obviously he/she doesn't get the whole amount as it also includes the room, sterilization and tools and Equipment related to the surgery.

Yes, it is very difficult for a doctor to earn 70 lakh per surgery he performs in any country but it's not impossible.
Doctors start with low salary then with time and experience they are able to earn more. In the beginning in Internship they start earning 40k-60k a month. Then after completion of Internship it depends whether they go for jobs or further studies which lead there future earnings.
There are surgeons like Heart surgeons , Diabetic surgeons , Neurosurgeons, Thoracic surgeons who earns more than 50 lakhs on critical surgery with over a experience of a 20-30 years in USA as well as other countries.

India is known for its low cost heart surgeries. So in private hospitals the charges for the Open Heart Surgery ranges in between Rs. 1.25-2.5Lacs in total. So do you think its feasible?
Such high amounts usually won't charged in any operation just by the surgeon. Even process such as Kidney transplant is done in an average fee of Rs. 5-6 Lacs among which the doctor charges around Rs. 1 Lacs. So such high amounts are not most probably charged by any surgeon.

The charge or the fees for a designated procedure is usually decided by the Hospital if the surgeon is attached to it. Now if he/she visits as a Guest Surgeon as per the request of the patients family, then the personal charges can be decided by the Surgeon. Although the charge is to be decided on the basis of the professional ethics and the average procedural cost. So its not like that a surgeon can ask a very large amount for a small procedure but usually its in average a charge which is affordable by the family and sometimes the surgeons from the point of view of humanity charges very feeble from poor patients.
