It depends on the policy and cost procedure of hospital that how much they are charging for surgery. Surgeon plays no role in charging cost of surgery. If, hospital is calling a guest surgeon on request of family then that surgeon may charge high price .
Cost of basic surgery in India is 2,25,000 and it can go up to 5 lakh . But cost of surgery also depends on type of surgery. So it can not be said that a surgeon can charge 15 lakh or not . You must contact hospital for these details.

Yes, ofcourse you can become Surgeon though you have eye power. Most of the surgeons wears spectacles after certain age. But the eye power must be in acceptable range.
But, you can correct the power by wear glasses. Wears glasses will not disqualify you from being a Surgeon.
Mostly you can wear spects and perform operation. Also, there is provision for operation to correct your eye power in some cases.

According to various reports the Navy, Army, Marine Corps and Air Force do not allow people into the military who have already undergone the procedure of Implantable Collamer Lens surgery . Radial Keratotomy (RK) surgery for correction of refractive errors is not permitted for any Air Force duties. Candidates having undergone cataract surgery with or without IOL implants will also be declared unfit.
Candidates who have undergone Photo Refractive Keratotomy(PRK) /Laser in Situ Keratomileusis(LASIK) may be considered fit for commissioning in Air Force all branches.
