Actually there are various hospitals in United States of America which charges different fee structure so there is not one common fee structure. And the charges goes up and down according to the requirements and critical situation of operation. I am mentioning below example:
Like Pali momi medical hospital in the Hawaii State of America charges $31,000 only for cardiac surgery including there is also a need of removing plaque from blood vessels. This does not include the fees of the doctor. However the average fees which are doctor charges for operation is 5000 dollars.
There are also various hospitals in United States of America which give discount to the patients for operation of cardiac surgery. The average discount which is offered is 30% to the patient for cardiac surgery.

Doctors start with low salary then with time and experience they are able to earn more. In the beginning in Internship they start earning 40k-60k a month. Then after completion of Internship it depends whether they go for jobs or further studies which lead there future earnings.
There are surgeons like Heart surgeons , Diabetic surgeons , Neurosurgeons, Thoracic surgeons who earns more than 50 lakhs on critical surgery with over a experience of a 20-30 years in USA as well as other countries.

• Candidate should be free from serious deformity and mal-developments. There should be no scoliosis, spina bifida and other skeletal deformities apparent or on clinical examination.
• There should not impede speech.
• There should be no sign of functional/organic disease of the heart and blood vessels.
• There is no fistula or evidence of piles.
• There is no disease of the kidneys, including glycosuria or albuminuria.
• There is no disease of the skin.
• There is no active, latent or congenital STD or HIV.
• There is no history of epilepsy and any mental and psychiatric disorder.
• There is no squint or any other morbid condition of the eye and eyelids.
• There is no active trachoma or its complications.
• There are no enlarged glands (Lymph nodes) in the neck and other parts of the body.
• There should be no evidence of pulmonary tuberculosis or previous history of this disease or any other chronic disease of the lungs. X-ray chest will be taken for this purpose.
• There should be no evidence of any disease of the digestive system, including any abnormality of the liver and spleen. Ultrasound exam (USG) of the abdomen will be done.
• There should be no inguinal hernia or tendency to hydrocele or definite varicocele or any other disease or defect of the genital organs.
Candidates who have been operated for the Hernia may be declared medically fit provided
• One year has elapsed since the operation. Documentary proof of this effect is to be provided by the candidates.
• The general tone of the abdominal musculature is good.
• There has been no recurrence of a hernia or any complication concerned with the operation.

The charge or the fees for a designated procedure is usually decided by the Hospital if the surgeon is attached to it. Now if he/she visits as a Guest Surgeon as per the request of the patients family, then the personal charges can be decided by the Surgeon. Although the charge is to be decided on the basis of the professional ethics and the average procedural cost. So its not like that a surgeon can ask a very large amount for a small procedure but usually its in average a charge which is affordable by the family and sometimes the surgeons from the point of view of humanity charges very feeble from poor patients. So this are very relative terms and nothing standardized figures can't be produced.
