See very first thing I would like to make your that in central institutions like NITs/IIIITs there is no reservation for state category and the category that you have mentioned gets reserved only on state levels until and unless mentioned in the central list of reservation available on the official website of NCBC.

Looking at previous year's cut off trends, for MBBS seat in government college, candidates from OBC category requires minimum 550+ score. Since there's increase in the competition cut off for MBBS keeps on increasing every year. With the score you have mentioned above it can be difficult for you to get admission in government college. Although you can try in subsequent or mop up rounds. You should apply through state quota if you belong to that particular state since 85% seats are reserved for State Quota. This might increase chances of your admission. You can also try to get admission in Semi Government or Private colleges, there might be decent chances of your admission. See on such marks you have very low or almost no chances to get any of the medical colleges under state quota of Telangana even after considering your OBC quota because for government seats the cut-off goes above 430 marks and for management seats there is no category reservation so for them the cut-off goes around 400 marks.
But on such score you can go for some Deemed Institutions through AIQ counselling, so as per the previous years data you can go for following deemed institutions through AIQ counselling :-
