You have to write ypur year of passing i.e. 2016 not the year in which you gave the improvement exam.And yes you are eligible to give jee mains as this is your last attempt but you can't give jee advanced now.So prepare well enough to get a good college like NIT or IIIT.

Candidates must have secured Minimum 75% marks as aggregate in class12exams or be in top20 Percentile from recognized board.
Post improvement exam, your aggregate score should be more than 75% .
You can select subject of your choice for improvement exam to score well above 75 percent.
Your age should not be more than 24 years while appearing in exam which is stringent eligibility criteria.
