You can select to apply for both merit-based and entrance exam based in a single form only even for the same courses.
You can apply for multiple programs/ courses through a single application.

The candidates are offered admission to the University of Delhi in two ways – Merit-Based and Entrance exam based. The registration is common for both and mandatory. Students can register for the Merit-Based and Entrance exam based through a single application only.

It would be rather advisable to fill a single form for both in order to avoid confusion in future.
