• All semester marksheets of BDS
• BDS Degree Certificate
• Internship Certificate
• Dental Council of India Certificate
• Authorized Govt. identity proof like PAN Card, Adhaar Card, Voter Id, etc.
• 10th & 12th certificate
• Category certificate (if any)
• Necessary documents required if applied under special category

• NEET MDS Result list 2019
• Marks Sheet of BDS(1,2,3 Professional exams)
• BDS degree or provisional certificate
• Certificate of completion of Internship from the head of the Institute stating that the Internship was completed before or on 31st, March 2019.
• A permanent or provisional registration certificate either from MCI or State Medical Council
• High School/Higher Secondary Certificate/Date of Birth for birth proof
• PAN card/Driving License/Passport/Aadhar Card/Voter Id for Id
• SC/ST/OBC/Disability certificate if applicable.
