Documents needed for document verification in NEET UG state counselling of Karnataka is as follows:

• Copy of Karnataka MBBS 2021 application form
• NEET 2021 Admit card
• NEET scorecard
• Secondary School Leaving Certificate (SSLC)
• Marksheet of 2nd PUC & 12th
• Two recent passport size photographs
• A study certificate which must be attested by Block Education Officer (BEO) or Deputy Director of Public Instruction (DDPI)
• Copy of bank challan
• Domicile certificate (if applicable)
• Reservation certificate (if applicable)
• Slip of application fee payment
• A duly sworn declaration regarding the mother tongue of candidates
