I have been at the same pace you have been and there were only 2 things that helped me.

1. alotting a table and a chair to study and one place only. if I was sore by sitting walking in the room. trying to keep the door open (if we are alone we will not study the ear of parents walking in might help you not use your phone for some time). read aloud and explain thing to yourself rather than just reading..like read your text and try to translate it to yourself in your mother tongue. and do not sit in one stretch you will forget half the stuff you studied.

2. get a group of friends and study together. divide topics and tell them to teach it to each other/ or at least explain it in their own words. all of you decide a portion to be ready with every day.

if your friends are not that kind, join in another group of people whom you might not know very well but want to pass the exams. in such cases, there will be less chitchat and more studying.

The best way to focus on studies and get rid of laziness is Meditation. Being a yoga expert, I would suggest you to do so. The best time to do meditation is early morning, as the atmosphere and environment is quite silent n peaceful in the early morning. Before sitting for studies, you can do mediation for 10 minutes, that'll help you more to concentrate and study for long hours. 

Switch off your mobile phone whenever you sit for studying as these gadgets divert us more and that breaks our link from studies. 

Once you start this, determine yourself and set targets, plan your schedule and prepare accordingly.
