Checking the  list of documents that are required for counselling are as follows

• Degree Certificate (Qualifying Examination)
• Class 10 ^th or its Equivalent Examination Mark Sheet.
• Study Certificate (10 ^th Class to Graduation)
• GATE/GPAT/TS PGECET Rank Card.
• TS PGECET Hall Ticket 2020.
• Category Certificate.
• Residence certificate.
• Transfer Certificate.

So, it is not mandatory to take application form.
