Depending on your percentile you will be given college. Since, FMS is the only college in DU offering MBA and it's a tier one college; therefore you need to perform really well in the exam.

FMS has its own admission form. What about other colleges like Delhi School of Economics, Department of Commerce etc?

Some of the other good Govt colleges for MBA are:
JBIMS, Mumbai
IIFT, DELHI
NITIE, Mumbai
IIT Bombay
IIT Delhi
Other old IITs including BHU
