For being able to be eligible for NEET 2021 candidate must have completed 17 years as on December 31, 2021. As you are born on June 2004 you will be 17 years of age by December 31st 2021 so yes you will be eligible for NEET 2021. The date fir the release of the application form is not yet declared. NEET 2021 exam is expected to be conducted in the month of July 2021 but there is no information from the officials yet. The dates for NEET 2021 exam or the application form is not yet released, Education minister Ramesh Pokhriyal 'Nishank' said that the application form of NEET 2021 will be released soon.

If you want to apply for NEET 2021, your D.O.B should be before 31st December 2003 . NTA clearly mentions this that the aspirations who are interested and want to apply be born on or before 31st December. If you are born in 2004 you can't apply for NEET 2021 . But can surely apply for the same from next year

There is no upper age limit for candidates appearing for NTA NEET 2021. However, an applicant must have to be at least 17 years old as on December 31, 2021. The candidate should be born on or before December 31, 2003. So, by the age factor you are not eligible for appearing for NEET 2021.
