Dear Surovi, this is not so important per se. But yes, please do mention that as a Prefix because you have a Death Certificate and it is correct to do that way. So to answer your question, it is not mandatory but it makes sense to add that infront of your Father's name. So that your mother or legal guardian can be considered as point of contact.
