yes you must keep these documents ready while the counselling for the OUAT 2018, 

along with the medical certs. hereby i am listing the documents that must be furnished. 

• Intimation letter-cum rank card
• Downloaded OUAT Form B application form
• Marksheet and pass certificate of HSC/SSC/Equivalent from the Board of Secondary Education/CBSE/ICSCE
• Marksheet and pass certificate of 10+2 level Science from CHSE/CBSE/ICSCE or equivalent
• CLC and Conduct Certificate form the Head of the Institution last attended
• Migration Certificate from the University/Board last attended
• SC/ST/Green Card/Physically Handicapped certificate form the competent authority for respective reservation category
• Certificate as proof of being OUAT employee from the Head of the office concerned
• Medical Fitness Certificate in prescribed form
• Residential certificate (Except for candidate with Odia as a subject in 10th standard)
• Aadhaar Card
• Permanent Residential Certificate for Western Odisha 
• and 5 passport size photographs
