Hello,
Yes it's necessary to upload a photograph with name and date present on it because it's written in the information bulletin of jee mains.However if you failed to do it,jee will not reject your application.You can also correct it in the month of january.If you haven't till now filled your jee mains application form then try to fill as early as possible because the last date to submit is 1st January 2018.

Yes it is necessary to upload the photograph with the date and the name on it.
I mean why did you want to risk your application? In future if they noticed this then they would certainly reject your application.
