see you need to contact with the school authority from where you have done the 10th course. or you need to directly contact with the exam authority of board of education under which you have passed 10th course. you need to related your problems and request to issue a duplicate marks certificate of 10th course. now do the form filling as per your right spelling of names. but in future you may face problem. so its better to correct the name on 10th certificate.

In the application form, your name should be filled as per the name you have on your class 12th mark sheet or passing certificate. So, if your name is spelt correctly on your class 12th mark sheet then you type out your correctly spelled name. Your class 10th marksheet name won't cause any problem.
