Since you have an affidavit it won't be any problem. They generally ask you to submit a copy of affidavit if such issues occur, so you don't have to worry.

Thanks for your kind reply.
But during the NEET application form fillup which name should I fill in the form the name which is in my 10&12 certificate(Babul Chowdhury) or the name which is in the caste certificate(Babu Kumar Chowdhury).I am very much confused and worried plz help.
