Sanjay, MCI has designated Disability Assessment Centers across India. Only assessments and certifications from these Centers are considered to consider you under PWD category and give you PWD reservation in NEET. It is not clear from where you got tested but you have to visit the nearest Disability Assessment Center listed on mcc.nic.in and get yourself assessed for an authentic PWD certificate. MCI rules state that any certificate other than from these Disability Assessment Centers is not considered valid for PWD reservations.
