In the medical fitness certificate, you have to mention the state rank. Here, the state rank refers to the overall rank ( the General Rank) that you have achieved in UPSEE, although you may mention your Category Rank in brackets(optional), and it does not make any difference because the Medical Fitness Certificate is to certify that you are medically fit to attend college, and it does not have anything to do with the rank.
