what should I do ,if there is no month or day mentioned on sports certificate ,it shows only the year . DU application form requires date.

You can fill the approx date you remember along with the month in which it happened.
Some things are required by DU and cannot be ignored.
Generally dates are mentioned on the certificate so check it properly.
