St. John's is a Private Medical College. Private Medical Colleges do not participate in All India Counselling and the seats are filled through State Counselling alone. And for appearing for State Counselling you need to hold the domicile of that particular State. Or else the only other option is through Management Quota. The eligibility criteria for seat matrix distribution:-

• Category 1: All India General Open Merit, includes all Indian Nationals irrespective of gender, religion, caste, community, region, or any such affiliations.
• Category 2: Institutional Staff Quota includes children of the permanent staff of St. John’s National Academy of Health Sciences who has served the Institution for ten continuous years or more.
• Category 3: Roman Catholic Christian Religious Sisters (Nuns), Brothers, Priests includes Professed Religious Women and Men who are members of Religious Congregations in India recognized by the Roman Catholic Church and Priests incardinated to Roman Catholic Dioceses of India.
• Category 4: Roman Catholic Christian Tribals includes Roman Catholic Christians who are members of recognized Tribal Communities.
• Category 5: Roman Catholic Christian Dalits includes Roman Catholic Christians who are members of recognized Dalit Communities.
• Category 6: Native (local) North Indian Roman Catholic Christians includes all Roman Catholic Christians who are neither Tribals (Category 4) nor Dalits (Category 5) and who are NOT members of the South Indian States of Kerala, Tamil Nadu, Karnataka, Andhra Pradesh, Telangana, Goa, Puducherry, Maharashtra and Gujarat, and who are NOT migrants or descendants of migrants to North India from the afore-mentioned South Indian States.
• Category 7: Roman Catholic Christians of Karnataka State includes all Roman Catholics Christians who have a domicile in the State of Karnataka as per the KEA criteria.
• Category 8: Roman Catholic Christians - All India Open Merit includes all Roman Catholic Christians of India who do not come under the reserved categories 2, 3,4,5,6, 7, and 9.
• Category 9: Differently-Abled Roman Catholic Christians include those Roman Catholic individuals with locomotor disability of lower limbs as specified by MCI norms.

Usually the governing body gives a window of about a week or so in between the allottment of seat and date of reporting to the college. If that time gap is present and you get enough time to attend the counselling in bangalore, and get the college of your choice, the other seat in Maharashtra might be reverted to the state itself. But if you take admission in maharashtra, that seat is taken what so ever and then you will have to pay the full fees for the entire coarse.

St. john's is a private institution. Since you have a score of 608, you can definitely expect a seat under open category which is open for all india. But mind you there are only 10 seats for it. However, with these marks you can apply for Govt medical colleges within Kerala as you will have advantage of state quota there.

If you are really interested, you can apply in St. john too as the previous year cut off went 600+ for open category students. Having said that, please note that the cut off varies each year by the seat matrix and the number of students appearing.

In St. Johns Bangalore there are only 150 seats for MBBS. And the cut-0ff score is  528 and the rank is 34157 for general category. As your NEET score is 498. Your Overall Percentile is:  95.98 - 96.48 and when it comes to rank, your Predicted Rank is : 53393 - 65543 . Also depends upon last years cut-off.

It’s difficult to be sure about the admission possibilities, the cutoff differs every year on the basis of various factors, It is also different for all India & state quota seats. The closing all india rank for St. Johns medical college Banglore in 2019 was 34157 . According to the 2019 trend for your score the rank must be around 45023-51086.
