St. John's is a Private Medical College. Private Medical Colleges do not participate in All India Counselling and the seats are filled through State Counselling alone. And for appearing for State Counselling you need to hold the domicile of that particular State. Or else the only other option is through Management Quota. So if you belong to Telangana, you should participate in Karnataka's State Counselling for admission to St. John's. Usually the governing body gives a window of about a week or so in between the allottment of seat and date of reporting to the college. If that time gap is present and you get enough time to attend the counselling in bangalore, and get the college of your choice, the other seat in Maharashtra might be reverted to the state itself. But if you take admission in maharashtra, that seat is taken what so ever and then you will have to pay the full fees for the entire coarse.

St. john's is a private institution. Since you have a score of 608, you can definitely expect a seat under open category which is open for all india. But mind you there are only 10 seats for it. However, with these marks you can apply for Govt medical colleges within Kerala as you will have advantage of state quota there.
If you are really interested, you can apply in St. john too as the previous year cut off went 600+ for open category students. Having said that, please note that the cut off varies each year by the seat matrix and the number of students appearing.

Your Overall Percentile is: 95.98 - 96.48 and when it comes to rank, your Predicted Rank is : 53393 - 65543 . Also depends upon last years cut-off.

It’s difficult to be sure about the admission possibilities, the cutoff differs every year on the basis of various factors, It is also different for all India & state quota seats. The closing all india rank for St. Johns medical college Banglore in 2019 was 34157 . According to the 2019 trend for your score the rank must be around 45023-51086. Hence it seems like it is difficult for you to get admission. However you may use the college predictor tool to know your chances.
