10th standard or preferably 11th standard is the best class when one should start preparing for this exam .

Well MD is itself doctor of medicine degree that students get after completely three years of MD program. You need to have mbbs to qualify for MD entrance exam and degree coursework. This is a postgraduate degree and is for choosing specialisation that the student want to pursue after undergraduate that is MBBS. Further studies can also be done. But you are already a doctor mbbs so automatically you will be a doctor after MD too.
