No,get the new one made,only the certificate which will be issued after 1st April2020 will be taken into account.
You get 596 marks which is good. And your ews rank also good. You get any medical college you want. Infact government medical college you get also.
This year cut-off marks is high but in Andra Pradesh 85% state quota expect that it will starting from 512.
But it's differ college to college so it's difficult to predict us that what cut off marks of your favourite college.

It is very difficult to get cse with if your EWS category rank is this not CRL rank . Also it depends on if you are resident of Delhi or not (home state Quota) . You should wait till last counselling or try for branches like ee , mech , civil et .

Hello Mam, Your rank is good and you also belong to EWS Category and that will be very much helpful to you.
Getting IIIT might be difficult but you can try for NITs and that will be very much helpful to you. You can even try for branches other than CSE at NIT and you can get that too.
I will recommend you to check All The Colleges, That is available for You on basis of your rankings and Reservation, Then You can also check that at Our JEE MAINS COLLEGE Predictor. According to the category rank you have mentioned in the question you cannot get any of the NITs or good colleges as per previous year's cut-offs until and unless you deserves Home State quota but I would suggest you to sit for some state counselling because in some of the state government colleges you can get admission on such ranks. Some college in which you can try are :-

Dear aspirant, The minimum eligibility criteria for admission in CCA for B.Arch is to score 50% marks in 10+2 with mathematics as one compulsory subject.
The selection of candidates in B.Arch is based on score of JEE main and merit list will be prepared on combined marks.
The candidates applying in EWS category first will be considered as General Ew category and during seat allotment they are considered as General category.

Unfortunately, he cannot use his EWS Certificate now if he had not filled his category as EWS in the application form of JEE-Main because the admissions will be made on the rank mentioned in your scorecard and if there is EWS rank then your cousin can use his certificate in Counselling.
