ICAR is conduceted for 15% seats in all government colleges throughout India so the concept of domicile might not have much effect.
Getting a rank below 1580 will get you a seat through ICAR, that is a score above 360 atleast. With yoour score of 238 the cahnes of you getting a seat through ICAR are not high however I will still recommend you to go through the counselling (till the last round).

As per the latest update on NEET-UG 2021 reservation criteria, the union government has decided to provide 27% reservation to Other Backward Class (OBC) and 10% reservation to Economically Weaker Sections (EWS) under the All India Quota (AIQ) scheme. So with the marks of 350 and also belonging to general EWS category, the chances of getting a seat in government medical college is quite low enough.
