This is an excerpt from the Information Bulletin:
Candidates will not be permitted to withdraw from the course after the last date of free exit notified by university. Candidates who have joined the course are not permitted to withdraw from the course after closure of admissions. If Candidates want to discontinue the course after last date of free exit/ after closure of admission they have to pay discontinuation fee of Rs.3,00,000/- (Rupees Three Lakhs only ) to the University and will also be debarred for three years from admissions in to same course in colleges under KNRUHS.
This means that you can only take admission in other seat by participating in further rounds of Counselling only if you have opted for Free exit. If you did not opt for Free exit but reported to the College, the chances of participating in subsequent rounds of Counselling are slim. And there is no policy of leaving the seat once reported. If you wish to do so, you will have to forfeit the College fees and also pay 3 lakhs in addition to the University. This is a decision that you need to take.

Predicting that whether you will get a government medical college or not with 400 marks required is quite difficult but when we go through previous year trends then
In neet 2019 , 426 marks was the cutoff marks for OBC category under Karnataka State quota to get a seat in Government Medical College and it was for Kodagu Institute of Medical Sciences, MADIKERI
Now,
There's 15% horizontal reservation for government seats for rural candidate and also 5 % reservation horizontal reservation for Kannada medium students.
But,
cut off marks for medical colleges keeps changing every year depending upon various factors like number of students that appeared in the exam, kind of marks obtained by them , toughness level of the examination, number of seats available etc.
So, considering all these factors including your reservation you should get at least 475 marks in your neet 2021 examination to get a government medical college.
And don't worry you still have decent time left for neet 2021 and you can definitely improve your score so that you can get a seat in a government medical college, focus on preparation, go well through the chapters , solve numericals and mcqs after finishing each chapter , follow a timetable to avoid wastage of time and once you are done with your neet syllabus focus on revision and then go through previous year question papers and mock test papers.

Your rank is not much good dear. So with this rank you can't get any government medical college. But you can get any private medical college. You know that this year cut-off marks is higher than previous year. So with counselling may be you can't get private medical college also but you can try it. But in management quota you can get seat very easily in private medical college.

So on such rank and category you have very low chances of getting government MBBS seat in any government medical colleges in Karnataka instead of that you can get government seats for MBBS in some private colleges under state quota according to this year's cut-off till Round 2 because till now the closing rank for the category which you have mentioned in the question is 100934 but yes if you go up to 450 marks in NEET then you may have shot at some government colleges but that as well for private MBBS seats. If you are desperate about a MBBS seat then don't compromise with that. If you are not that passionate towards MBBS seat then opt for BAMS. If you are determined about a MBBS seat then go for repeating. Let me put this clearly many students who have secured a seat this year have been taking NEET for the second or even third time. Even the one who got 720/720 was taking NEET for the second time. So taking NEET again might turn fruitful but that completely depends on your effort. You can wait till mop up round because having hope is never wrong.

You can participate for round one counselling in both your state and all India counsellings and if you do not get a college of your choice you can proceed to round two in any one(either AIQ or state counselling) based on where you think u have more chance of getting a seat
