According to the previous year cut off, an OBC category candidate scored 539 marks to fetch a seat in government medical colleges in Tamil Nadu, which is your domicile state.
When we look at the cut off's of other states too, they go as stated under, for OBC category-

Madhya Pradesh- 545
West Bengal- 574
Rajasthan- 630
Uttar Pradesh- 597
Bihar- 606
So you can see you have very low chances of getting into a government college, or even securing government quota in private colleges. However, you do have chances of getting the following private and deemed colleges-
Heritage Institute of Medical Sciences, Varanasi- 397
Hind Medical College, Sitapur- 398
Muzzafarnagar Medical College- 398
BLDE University, Bijapur- 367

If you scored 250 marks in NEET enterance exam then I suggest you to go for BAMS course which is eligible for you, according to your marks scored in NEET-UG.
Because BAMS required 113 marks in NEET UG for General catagory and for ST/SC/OBC marks required is 86 marks.
So, You can easily go for a course in BAMS government colleges in which counseling procedure is done in 2 Catagories such as
1. All india Quota AIQ with 15% seats for reservation criteria students such as ST/SC/OBC which is more than unreserved students like General catagory
2. State Quota in which remaining 85% seats available which varies from state to state.

It is difficult to determine which colleges you can get for your NEET UG score because cut-offs changes from year to year a nd depends on many factors like
1. Number of seats available.
2. Difficulty level of the exam.
3. Number of students appeared in the exam.
4. Caste Category of the students.
It is difficult to get free seat for your NEET UG score based on previous year's cut-offs.

As you belongs to general category 460 mark is not enough to get admission in government medical college. You need to score minimum 600+to get admission in government medical college. This is based on previous year cut off. As you have already mentioned that you are late for bca admission,it means you don't have chance to get a college of your choice.
So according to my opinion you should wait for 1 year and also in this time period you should also prepare for neet 2023,with hard work and good luck may be you got good marks and alot a seat in good college.
Don't waste your time, you have time do something creative.
Neet cut off vary year by year. It depends on various factors like toughness of exam, number of candidates appeared, number of seats available etc. 2021 cut off for general category was 720-138. That is 50 percentile. For sc/st/obc it was 137-108. For UR pwd cut off was 137-122.That is 45 percentile. Sc/st/obc pwd : 121-108.
Now coming to 2020 cut off. General category : 720-147. Pwd general category : 146-129. Sc/st/obc : 146-113. Pwd sc/st/obc : 128-113.
So expected neet cut off will be around this range.
If you scored 220 marks in NEET and you don't want to take a drop year then you should go for BAMS- Bachelor of Ayurvedic Medicine and Surgery course which is good for you and it's duration is 5.5 years and Admission taken on the basis of NEET enterance examination marks.
So eligibility criteria for Admission is you have scored minimum marks required for General catagory is 113 marks and for OBC/ST/SC catagory is 86 marks required in NEET.
I suggest you this course because in other courses you need to score more than that like 350+ or 400+ and also you don't want to take a drop year so, according to your marks you can go for it BAMS government colleges.

If you score less marks or if you do not qualify for MBBS or BDS you should go for BPT/BHMS/BAMS. If you do get admission in that course as well you should opt for B. Pharm.
