See for Dam affected candidates of Maharashtra, there is no reservation in NEET UG, so you should opt for General category in NEET UG application form until and unless you belongs to any of the EWS/OBC-NCL/SC/ST category.
Complete NEET UG 2022 Application process is explained below in very easy steps :-
** Very first step is to visit the official website as given above and click on "New registration" button.
** Then you have to do registration using your basic details such as name, father's name, mother's name, contact details and so on.
** Now on second menu item you will have to fill details like your academic performance, address, ID type, medium of question paper and so on.
** Then on Next menu item you have to upload some images such as :-
i. >>>>>>> The recent photograph should be either in colour or black and white with 80% face (without mask) visible including ears against a white background.
ii. >>>>>>> Scanned photographs, signature, and Left and Right hand Fingers and Thumb impression should be in JPG format (clearly legible).
iii. >>>>>>> Scanned Category Certificate (if applicable), Citizenship Certificate (if applicable), and PwD Certificate (wherever applicable) should be in pdf format (clearly legible).
iv. >>>>>>> The size of the scanned passport photograph should be between 10 kb to 200 kb (clearly legible).
v. >>>>>>> The size of the scanned Postcard photograph (4”x6”) should be between 10 kb - 200 kb (clearly legible).
vi. >>>>>>> The size of the scanned signature should be between 4 kb to 30 kb (clearly legible)
vii. >>>>>>> The size of scanned and Left and Right hand Fingers and Thumb impressions as per the Template provided (Appendix-XXIV) should be between 10 kb to 200 kb.
viii. >>>>>>> The size of the scanned copy of the Category certificate (SC/ST/OBC/EWS etc.) should be in pdf between 50kb to 300kb (clearly legible).
ix. >>>>>>> The size of the scanned copy of the PwD certificate should be between 50 kb to 300 kb (clearly legible)
x. >>>>>>> The size of the scanned copy of Class 10 pass certificate should be between 50 kb to 300 kb(clearly legible).
xi. >>>>>>> The size of the scanned copy of the Citizenship Certificate/Embassy certificate or any Documentary proof of Citizenship certificate should be between 50 kb to 300 kb (clearly legible).
** Then you have to proceed for payment of application fee which can be done only in online mode either through Credit/Debit card or UPI or NEFT/RTGS.
** Then after you have to take the print-out of the NEET Acknowledgement receipt.

Do not worry, there will be no problem regarding your name entered in your NEET Application form. You can Correct it when the NEET Correction window opens.
The board will provide the facility to make correction in the application form . It will be done from the month of May 2022. The candidates must login to the website and verify their particulars and make necessary corrections wherever required.
Further, any incorrect information entered in your NEET application form, NTA authorities will be notifying you in your Email ID and then you need to correct the same information when the Correction window opens up.

If you have mistakenly attached any document rather than the class 10th certificate, then there must be any discrepancy raised at the time of verification, there is always an edit choice option or the correctional window to edit your updated documents, keep yourself updated with the opening of the correctional window notification, and from the next time be sure before you enter any document.

NEET UG registration cannot be cancelled but don't worry you can change your category with correct category during NEET UG 2022 correction window which will be released after the application process gets concluded i.e. after May 6th, 2022.

You need to choose the code 02 while filling neet 2022 application form
This is because,
Though you are doing Bsc your highest qualification with all the three subjects i. e physics chemistry and Biology/biotechnology remains your 10+2 as you haven't passed your bsc 1st year yet.
While, those who have passed 1st year of 3 year graduation with all the three subject i. e physics chemistry and Biology/Biotechnology needs to select code 5 and those who have completed BSc with at least any two of physics chemistry and Biology in their bsc need to select code 6.

-----------------------------) Code 1 :- code 1 is for class 12th board 2022 examination appearing students with physics, chemistry, biology and English or have have already appeared but results are awaited i. e have not been declared.

As your category is listed in central list of reservation of OBC so you should fill your category as OBC-NCL in NEET UG 2022 application form.
NOTE : --- OBC-NCL certificate that is to be uploaded in the NEET UG 2022 application form, should have been issued on or after January 1st 2022 according to the latest guidelines of Government of India. In case you fail to upload it (issued on or after January 1st, 2022) at the time of registration then the OBC candidate has to upload a declaration [Declaration in Lieu of OBC-NCL Certificate as per Appendix - XB in Information bulletin] to that effect. See nomadic tribes are reserved under OBC category at national level but you should once check the central reservation list available at official website of NCBC and if your category is listed there then you should opt your category as OBC-NCL in NEET application form otherwise kindly fill your category as General category.
If you belongs to OBC-NCL category then you should also upload your OBC-NCL certificate.

In your case you should fill your number of attempts as 0 and in passing status you should select as appearing for class 12th, there is no need to mention anything about board examinations of 2021 because you got failed in that so kindly mention the details of board examination 2022.
As you are appearing this year for you 10+2 examination so kindly fill your NEET UG qualification code as 01 because results are not yet out and even exams are to be conducted.
See as per the instructions given by NTA for NEET UG 2022 the name that is to be filled in NEET UG 2022 application form should be as same as mentioned in class 10th passing certificate so you should correct your name in the correction window however it won't create any problem but there is also no any harm in following the instructions given by the agency.
