You haven't mentioned your category, as each one has different cut-offs, anyway presuming you to be General, last year the least marks required to secure a GMC seat was 577 marks for Government Medical College, Bettiah and you are short of merely few marks, but you may get other colleges such as Katihar Medical College, Lord Budha Koshi Medical College and Hospital, Madhubani Medical College,Narayan Medical College and Hospital( Sasaram)in case you belong to SC/ST there are greater chances of getting GMCs through state quota as well as AIQ.

If you belong to General Category, 230 is a less score even for a seat in BAMS. It is not that Candidates who have lesser scores in NEET apply for Ayush Courses. These Courses also have cut offs atleast at 450 if not 500 or 550 like MBBS/BDS. There is a huge demand for these courses as well and so the cut offs are usually higher. So with a score of 230, you might get a Management Quota seat in Private Medical College but Govt. seat is difficult. Having said that, this is based on 2019 data and cut offs change every year. Cut offs change every year depending on various factors such as no.of Candidates appearing for the exam, no.of Candidates qualified, top NEET score and difficulty of the exam. See it is little bit difficult to predict exactly whether you will get seat in any government medical college or not because different colleges have different cut-offs for State quota and All India Quota. Also, cut-offs are bound to change every year depending upon various factors such as the number of candidates appearing in an examination, the number of candidates qualifying an examination, difficulty level of the paper, and so on.
But if we go through previous years NEET cut-off trends in Rajasthan then there are good chances for you to get seat in some of the GMCs, the reason being is last year the lowest cut-off marks was 564 for Government Medical College, Barmer for Management quota seat under Home State quota for OBC category.

so as the eligibility says, you must have 60% aggregate in class 12. so it seems you cant get admission.

you have really tough chances to get into any government colleges as per previous year analysis and your rank.because you are just near to cutoff for securing a seat you need to score well above cutoff.as of now its tough to say which college you can get into beause the cutoff changes every year.but you have low chances to get into private colllegs also.you can try through management seats or take a longterm if you are interested.for furthur info check the following links:

You do not have to really worry about the medical fitness certificate as
The medical college you are going to get in counselling have its own teaching hospital
And you will have a medical check up there itself at the time of admission and do not worry it's just a formality unless you have really big issues such has handicapped , colour blindness , etc
And trust me , you will enjoy your check up as you will first time see your hospital , you will be treated as special guest , you will stand out of line , will be attended first and completely free , you will get a feel of a doctor
And in case you want to be too sure , you can get fitness certificate from any Gov. Hospital .
