Yes, you have to fill the Certificate no.3 given in the Brochure of UPSEE as it is a Proforma For Certificate Of Domicile of Uttar Pradesh, This is advisable that you should fill that form which proves that you are a permanent resident of Uttar Pradesh and get it attested by respective authorities.
The Domicile Certificate issued by State Government is also used for the same purpose but The validity of that domicile certificate is of 3 years and after the tenure of 3 years you need to renew it. So that's why UPSEE has given that certificate in the Brochure which proves that you are a Current Permanent resident of U.P.

At a neet score of 555 you have chances at government quota seat in bihar counselling as the cutoff is around 420+ but also as this year cutoff has increase significantly.For delhi the cutoff will be around 600+ this year.

to get around 50,000 rank in JEE Mains, you need to score about 115-120 marks and for 15,000 rank you need to score close to 150-155 marks (all approximations based on past year trends analyzed by coachings and websites)

As per you have mentioned that you want to get in in medical line, then you have to give neet examination, and for that you should be prepared with both 11 and 12 syllabus.
However as a answer to your question if you want to do science from NIOS after 11 humanities, you have to start from 11th science you cannot skip the the oneplus and go to another without completing 11th final examination. Yes, you can join competitive exam coaching classes from 11th. If you wish to prepare for UPSC, then you may join any coaching classes. Mist of the students now a days, join coaching classes compulsorily for the studying about UPSC.
But, remember, it must hamper and come between your daily study schedule mostly. If it comes, then your 11th and 12th would not be good and you could end up getting low grades in your 12th examination.
You can also start preparing for UPSC after 12th, there's no compulsion that you have to start studying and preparing for UPSC after 11th. I am just saying this because, you will only get a good college for graduation if your 12th grades would be good.
But, if you can handle both, 11th std studies and going to coaching classes then it's fine and OK.
There are various platforms available from where you can study and gain knowledge about this competitive exam.

You can only to be know as someone from punjab domicile only if your I'd proof I.e your Aadhar card proves it that you belong from there. Otherwise getting education from there in a boarding school doesnt make any difference. You will only be known from punjab domicile if you prove it to belong from there through your identity proof I.e Aadhar card.

The eligibility criteria for enrolment in the BDS degree course is 10+2 with Physics, Chemistry, Biology with at least 50% marks (40% for reserved categories). BDS Admission is through qualifying the competitive examination NEET (National Eligibility cum Entrance Test) only.
For getting BDS in any government colleges, you need to get a rank between 3792-10920 for general category in NEET UG examination. For government colleges in Kerala, the fee structure is about 1 lakh or less for the whole course. For private colleges in Kerala, the fee can go from 12-24 lakhs with respect to colleges.

The National Institute of Open Schooling (NIOS) has instructed every state University or college to accept admission on the basis of score or marks provided to students in the open school.
So I guess you are free to apply to colleges you are interested on the basis of your class 12 marksheet issued by the respective open school.
