If your name appeared in the first admission list of DU LLB released on July 25, 2018, then you must have also recieved the call letter for the counselling and document verification which took place between July 25 to 28, 2018 (the last day was extended by one day).

With regards to the admission form, yes you are required to fill the admission form, but you need not send it to anybody, rather you are required to take it with you to the document verification.

Since you haven't taken any steps after the release of the first admission list, I suggest you wait for your name to appear in the second admission list (to be released tomorrow, i.e. July 31, 2018) and then take action accordingly.
