If you want to change your field of interest from engineering to medicine then you should go in the profile section and their you can change your field of interest from engineering to medicine.

Normally the change of option from engineering to medicine in the app is done through the settings. You have not mentioned  the app for which you are talking, so I can't tell you about the process of changing the options. So please mention the the name of the app next time when you ask the question. If you are talking about career 360 app then the changes can be done by going to your profile on the app and then can change your option.
