Yes, you need to upload the original documents in the aplication form of NEET 2021.  You will require the following documents for NEET 2021 application form:

• passport size photograph
• class 10th certificate/marksheet
• left thum impression and signature

If you have lost your original 10th class marksheet then you can download it from the DigiLocker. It is official government site, you can download the marksheet by entering you adhar number. For more information, visit here:

You need to upload a scanned copy of your original 10th marksheet and you can not upload the Xerox of your 10th marksheet.

It is a must to upload all the original documents for form filling of NEET. All the original documents are needed to be scanned and the uploaded accordingly.
