You cannot mark Code 6 as you not completed your B.Sc. Degree Program, you will have to mark Code 5. We assume you have passed your first year exam but not taken the second year examination, this is what we understand from your question, and hence you need to mark Code 5.
Code 5, states that anyone who has passed their first year of a Bachelor’s Degree from a recognized university with Physics, Chemistry, Biology and that examination is a university level examination. Also their 12th standard educational qualifications are also with Physics, Chemistry, Biology and English as a compulsory subject.

If you have uploaded the picture as same as the dimensions of passport size picture of yours than there are chances that you might not need to make changes again in the application form of NEET2020. But if the size of photograph does not meet the criteria as mentioned in the instructions to fill the form then you need to make the changes.

NEET entrance is the entrance test for medical aspirants. You are in BA first year . Do you have Science background ie if you did your xii board in science stream and you must have minimum 50% marks in PCB Subjects then only you will be eligible for appearing NEET. Arts students are not eligible for neet exam.

see you are eligible for state counselling in the state which you have mentioned in neet ug application as for your domicile criteria, also you must have the required documents which supports you the state domicile eligibility. for any others state colleges you need to participate in all india counselling.
We know that you are quite young and it is ok to be apprehensive. But trust the system on this. This will not be a problem at all. No Candidate will be denied an admit card or entrance to an examination hall just because the middle name is different. You did not receive notification from NTA asking you to make changes. That is the first sign. Then you went ahead and also made corrections to the application form. That is the 2nd sign. These 2 signs are in your favour. Now let's say you are an authority and I am a student. By mistake I write my name as Prianka Sarkar instead of Priyanka Sarkar on the Aadhar card. But the rest of the details like 10th marks sheet, application form and all other valid Id proofs have Priyanka Sarkar. Wont you consider the rest of the proofs because the Supreme Court has mentioned that Aadhar Card is not mandatory for admission and entrance examination purposes? If you hold me back, wont it be going against the Supreme Court's ruling? This is the same case with you right now. You should not be worried because there is nothing to be really worried about. What you should be worried about is being worried about the admit card, you are losing out on the time that you could rather spend preparing for the exam. So do not worry and prepare for the exam. Last thing you want to do is worry about the admit card and lose out on securing a MBBS seat.
No you will not face any problem in examination centre, these much of changes in name are acceptable, but if you want to be super safe then take copy of your father's aadhar also ,I have experienced this in my exam , they said that these much interventions are acceptable

National Testing Agency (NTA) use to conduct NEET exam and for this year they closed the application for NEET 2020.
You need not to worry for this issue if you have not received any mail or notification regarding any mismatch in your application form. As NTA verifies on their own for each and every application and if any mismatch or any discrepancy is found they notify the candidate about that and if you did not received any such kind of mail, than you are safe and your application has been registered successfully.
If there will be any problem in counseling or in any other process after exam than you can use affidavit.

see there is no age limit to appear in neet ug exam, and if you do not have biology in 12th course you may take nios exams and nios certificate is accepted by all government and private organization in india, now if you cleared the compartment paper before the admission of neet ug then it is acceptable. the qualifying marks criteria for neet ug is 50% aggregate of pcb for general category students, 40% aggregate marks of pcb applicable for sc/st/obc candidates.
