Every year JoSAA releases the format for various documents among which the format for the MEdical Certificate is also included. You have to download the Business Rules file from JoSAA 2019 official website and search for a certain annexure which contains the entire required format for the Medical Certificate. Take a printout and fill all the details correctly and carefully and get it signed by the doctor as well.

I am very sorry to say that you will not be allowed to sit for JOSAA counselling as you haven't qualified for advanced. This year only the qualified candidates are allowed to sit for the counselling conducted by JOSAA. You can try in private colleges that accept JEE main score or you can sit for the state counselling. Congratulations for the JEE main results.

Yes you can apply for JoSAA councilling even if you have not cleared JEE advanced as JoSAA not only allots seat in IIT but it also gives allotment to NITs and IIITs where allotment is based on the JEE main results and with 94 percentiles you will surely get a NIT . So you must apply for JoSAA councilling and from this year ews category is also introduced so that will certainly benefit you.
