Manufacturing Science and Engineering is a study of Mechanical Engineering and is not a separate department. The distinction between the study of Mechanical Engineering and Manufacturing Science and Engineering is not strong. These courses include Thermodynamics, Fluid Mechanics etc.
Regarding the future of this study, the placement status is not very encouraging. In fact, it's bad. If you are a student who would like to continue your major career, the only option you have is to fly abroad to get a higher education. If you are planning to pursue higher education, this is a good course
Although you may have to slightly compromise on the branch, I believe Chemical at IIT Bombay is a better Option compared to Instrumentation at IIT KGP. Placements, Faculty, Infrastructure, Campus Life of both options are comparable, but IIT Bombay has a huge competitive advantage, being located near Technology and Finance Hubs of Mumbai and nearby Pune, hence huge scope and impetus to training, projects and placements.

Well, 289 is a very secure and safe rank in JEE advanced.
Eligibility criteria for qualifying jee advanced:
Minimum 75% should have been secured in 12th standard (or equivalent) examination. For the SC/ST/PwD category, the aggregate marks is minimum 65% OR. The candidates should be within the top 20 category percentile of the qualifying examination.
Other than this, each every document should be clear and original. Even if you secure a good score in JEE advanced but there is some issue with the documents, it might lead to disqualification of your name.

Yeah! For the specialization you will be asked to choose the corresponding course from your 3rd year at IIT Kharagpur. As you are in Dual degree program, you will have to do a project (MTech project) which will essentially be on your specialization.

Undoubtedly ECE Dual Degree at IIT Kharagpur has much better career scope and placement prospects than Civil at IIT Madras. ECE as a branch is quite interesting, and effectively is a wide branch with verticals extending to Computation, Electrical Technology, Material Science, Automation & Robotics and nowadays IoT and AI.

It is completely your call dear if you want to to get into IIT Kharagpur then you can take a drop and prepare again and clear JEE advanced next year and can get into IIT Kharagpur depending upon your rank but if you are ok with NITs then with your rank you have decent chances in NITs so you can choose not to take a drop or you can also so take partial drop means you can take admission in NIT and along with your btech side by side you can prepare for jee advanced.
So, take time, think over what actually you want and then make a decision.

I hope you are doing fine, Go for BTech. I think the dual-degree programs are fantastic, but why go for an early commitment. And if the MTech part of the dual degree is in a specialized area, that should be avoided.
Coming on to the college I would Suggest you to go with IIT Delhi since it is in Metro City and Many Companies go there for placement that do not go to IIT Kharagpur.
better professors, the caliber of your would-be batchmates, placement opportunities (A lot of multinational corporations like Samsung Korea and Rubrik U.S., don’t visit IIT Kharagpur, but regularly visit IIT Delhi to hire students for internships and placements), as well as the opportunities for pursuing further studies from top-tier international universities.
